Key Responsibilities

• Monitor and document patient vitals, symptoms, and health status
• Administer and manage medications; pre-fill medication boxes
• Provide care for G-tube feeding, Foley catheters, and suprapubic catheters
• Perform wound care per physician orders
• Manage tracheostomy and ventilator care when applicable
• Provide suctioning and respiratory support as needed
• Maintain accurate and timely charting and documentation in compliance with agency and state standards
• Communicate effectively with patients, families, and interdisciplinary teams
• Maintain a safe and hygienic home environment in line with care protocols
• Provide emotional support to patients and families facing chronic or critical illness
• Prive Duty LVN. Must be comfortable with administration of medications through feeding tube

Qualifications

• Current LVN or RN license in the State of Texas (required)
• Valid CPR Certification (required)
• At least 1 year of clinical nursing experience (preferred)
• Home healthcare or Private Duty Nursing experience is a plus
• Proficiency in tracheostomy care, ventilator management, G-tube feeding, and catheter care
• Wound care experience (preferred but not required)
• Ability to work independently and manage care plans with confidence
• Excellent communication, assessment, and critical thinking skills
• Willingness to work some evenings, weekends, or holidays as needed
• Reliable transportation and punctuality

Schedule: Private Duty LVN 24 hours per week 9-5 Friday, Sat and Sun.
