LPN Home Health PRN

Empath Home Health, a member of Empath Health is seeking a compassionate PRN Home Health LPN to work with our Home Health team to deliver individualized care to patients in their homes in Manatee county.

Locations: Home health visits in Manatee county (including Bradenton, Lakewood Ranch, Anna Maria Island, Ellenton, Palmetto, and surrounding areas)

Schedule: PRN; 5-10+ home health visits per week.

What you'll Do

• Provide nursing services in compliance with agency policy and procedure, state and federal regulations, and the patients plan of care under the direction and supervision of a Registered Nurse.
• Observe patients and report adverse reactions to medication or treatment to the Clinical Coordinator.
• Assist the Physician and/or Registered Nurse in carrying out the plan of care and in performing specialized procedures.
• Perform patient and family instruction on diet, medication, and treatment.
• Collaborate with the interdisciplinary team to ensure seamless, holistic care

What You'll Need

• Active Florida LPN license
• Applicants must be a graduate of an approved nursing program.
• Minimum of 1-2 years of experience as a LPN.
• Home health care experience preferred.
• Strongly prefers experience using Homecare Homebase (HCHB) EMR
• Intravenous (IV) Certification strongly preferred.
