PRN Home Health Physical Therapist

HCA Healthcare seeks a dedicated and patient-focused PRN Home Health Physical Therapist to provide top-tier physical therapy services in a home health setting. As part of our team, you will help to enhance or restore function, alleviate pain, and prevent disabilities following disease, injury, or loss of a body part. This role involves traveling to patients' homes to deliver direct care, guiding them towards a journey of recovery and optimal health. You will report to Home Health Managers and work collaboratively with a multidisciplinary team dedicated to improving patient outcomes.
Assess patient condition by conducting physical and psychological analyses
Develop tailored treatment plans based on patient assessments and goals
Implement physical therapy interventions to improve patient mobility and reduce discomfort
Educate patients and families about proper safe handling and movement techniques
Consult and collaborate with other healthcare professionals to promote patient health and well-being
Keep accurate documentation of patient status and progress
Maintain patient confidence and protect operations by keeping information confidential
Proficiency in designing effective treatment plans
Strong interpersonal and communication skills
Ability to handle physically demanding interactions
Expertise in using therapeutic equipment and techniques
Knowledge of healthcare software and systems
